In addition to the university's vaccination requirements, listed on Immunization Compliance Overview page, 91视频Health Professional Graduate student may have additional requirements. Per 91视频Policy 33-002, it is mandatory for 91视频students to submit all required immunization documentation and/or the completed 91视频Medical History Form prior to course registration. Course registration will be restricted until you have fulfilled this requirement.
